    Data Protection Officer (DPO)

    The Controller has appointed a Data Protection Officer, who can be contacted by email at: iod@adm.uw.edu.pl.

    You may contact the DPO on all matters related to the processing of your personal data by the University of Warsaw and the exercise of your rights under data protection regulations.

    The DPO is not responsible, however, for handling other issues such as providing information on forms of assistance offered by the Office for Persons with Disabilities of the University of Warsaw.

    Purposes and legal bases of processing

    Your personal data will be processed in order to provide assistance by the Office for Persons with Disabilities and to ensure that persons with disabilities have conditions for full participation in the process of admission to the university for education (including ensuring the possibility of studying under an individual study arrangement), in the course of education and research activities. The documentation you submit forms the basis for using all forms of support offered by the Office for Persons with Disabilities to candidates, students and employees of the University of Warsaw with disabilities and health difficulties.

    The processing of your personal data is necessary to comply with the legal obligations imposed on the University of Warsaw in connection with the implementation of tasks specified, among others, in the Act of 20 July 2018 – Law on Higher Education and Science (consolidated text: Journal of Laws of 2020, item 85 as amended), implementing acts issued on its basis, and internal acts in force at the University of Warsaw, as well as in the Convention on the Rights of Persons with Disabilities of 13 December 2006 (Journal of Laws of 2012, item 1169) (Article 6(1)(c) GDPR).

    Your personal data may also be processed on the basis of the legitimate interests of the controller, i.e. the right to establish, pursue and defend legal claims (Article 6(1)(f) GDPR).

    All other personal data not required for the above purposes may be processed on the basis of your separate consent (Article 6(1)(a) GDPR).

    The legal basis for processing special categories of personal data (health data) is Article 9(2)(a) GDPR – consent to the processing of special categories of personal data. Consent may be withdrawn at any time, e.g. by sending an email to: bon@uw.edu.pl. Please note that withdrawal of consent does not affect the lawfulness of processing carried out on the basis of consent before its withdrawal.

    Data retention period

    Your personal data will be processed for the period necessary to achieve the above-mentioned purpose of providing necessary support by the Office for Persons with Disabilities. In the case of the admission process – for the duration of the admission procedure, and after its completion for 6 months; in the case of admission to studies – for the duration of studies, and then they will be archived and stored for 50 years.

    Where applicable, your personal data will be processed for the period necessary to establish, pursue and defend claims.

    Recipients of data

    Your personal data will be accessible to authorised employees or associates of the University of Warsaw, to the extent that access is necessary for the performance of their official duties.

    Recipients of your data may also include entities commissioned by the Controller to perform specific activities involving the need to process personal data. In order to ensure the highest possible standard of data protection, data processing agreements will be signed with these entities.

    Transfer of data outside the European Economic Area (EEA)

    Your data may also be processed by the provider of the Google Workspace for Education Fundamentals service – Google (with whom we have signed a data processing agreement) – in its data processing centres.

    Rights related to data processing

    We guarantee the exercise of all your rights as provided by the GDPR, i.e. the right to:

    • access your data and obtain a copy thereof;

    • rectify (correct) your personal data;

    • restrict the processing of your personal data;

    • erase your personal data (subject to Article 17(3) GDPR);

    • lodge a complaint with the President of the Personal Data Protection Office if you consider that the processing of personal data violates the law on data protection.

    Obligation to provide data and consequences of failure to provide data

    Providing your data is necessary to ensure conditions for full participation in the admission procedure for studies. Providing other data, which are not necessary for ensuring conditions of full participation in the admission procedure for studies, is voluntary.
