Transportation

All students who would like to use special transport and the assistance of the assistants in moving to classes, we inform that the transportation questionnaire must be completed electronically through the form posted on the website of the Office.

To the Office the student should provide:

• A certified copy of the current certificate of disability (if previous has expired),
• Medical documentation confirming the current health conditions (only the students applying for the transport for the first time and those whose health conditions has changed).

The questionnaire must be completed electronically and delivered personally or by letter to the Office.
